2. Raffle tickets will be given at the check - out counter.
3. Simply write your name , address, telephone no., and signature on the raffle coupon. Drop in drop boxes at any CSI Branches near you.
4. Winners can only win once. Prizes should be claimed personally by the winner on the awarding date schedule by the CSI Management. Once the winner was not able to attend the said schedule of awarding of prizes they should wait for another schedule.
5. All winners will be notified thru Text. A list of winners will be posted in all CSI outlets and announced at USATV.
6. Prizes are not convertible to cash and subject to 20% tax. Registration and Insurance Policy of the vehicle will be shouldered by the winners.
7. Winners should personally claim their prizes within sixty (60) days after the date of announcement. Any unclaimed prize 60 days after the receipt of notice shall be forfeited upon approval of DTI.
8. All CSI and Sponsors employee, including their relatives up to the 2nd degree of consanguinity and affinity are disqualified from joining this raffle promo.
9. Raffle draw date will be on April 1, 2023 at CSI the City Mall, Lucao District, Dagupan City. Deadline for submission of entries is on March 26, 2023, 6:00 PM. Raffle stubs not dropped on the said deadline date will no longer accepted.
